Pre-Marital Coaching Form

Thank you for your interest in having FIVE39 Church be a part of your big day!

Please fill out the below questionnaire as completely as you can. This questionnaire allows us to get to know you better and sets the framework for your future pre-marital coaching sessions. Please note that on this form there are questions for both the Bride and Groom to fill out separately.

After you submit this questionnaire, you will be contacted by one of the Pastors on our team. The Pastor will reach out using the email and/or cell number you provide. It is up to you to reply once the Pastor reaches out. If the Pastor does not hear back from you, he will assume you are no longer interested in having FIVE39 Church conduct your pre-marital coaching.

The 2025/2026 cost for a wedding is $900, which includes 4-6 hours the day of the ceremony in the Auditorium and 1 hour the day before the ceremony for the rehearsal. This also includes the cleaning fee and use of a sound tech and wedding coordinator.

All other specific details will be given at the first meeting.

The FIVE39 marriage process consists of completing class settings and a 1:1 meeting with your officiant that stretch over a minimum of six weeks. Are the two of you willing to attend the FIVE39 pre-marital coaching classes/sessions? *

These sessions are typically offered twice a year (fall & winter). Topics covered include but are not limited to, biblical roles, conflict resolution, spiritual & sexual intimacy, finances, communication, in-laws, etc.
